Privacy Notice
How we handle personal information on ivaccinated.org, in plain language.
1. Who we are
ivaccinated.org (the “Site”) is operated by Alliance for Health Implementation Science, Inc. (“AHIS,” “we,” “us”), a Delaware nonprofit corporation based in Thousand Oaks, California. AHIS is responsible for the personal information collected through the Site. Questions: info@ahis.org.
2. The short version
- Looking up your vaccine record: we collect nothing. You go directly to your state or territory’s official registry. We never see, receive or store your record.
- Asking for a sticker: we collect your name, email and mailing address, and use them only to mail stickers.
- We never ask about your health or your vaccines, and we don’t sell your information.
3. What we collect
Sticker requests for yourself: first and last name, email address, mailing address, and whether you want occasional updates.
Sticker requests for a provider or pharmacy: your first and last name and email address, the provider or pharmacy’s name and mailing address, and, if you choose, the name of a contact there.
Email: anything you send to info@ahis.org.
Information collected automatically. Like most websites, our hosting provider keeps standard server logs, such as IP address, browser type, pages requested and the date and time, to operate and secure the Site.
4. Not health information
The Site does not ask about your health, your medical history or which vaccines you have received, and the state picker does not record which state you choose. A sticker request is a request for a sticker. We do not treat it as, or use it to infer, information about your health or vaccination status, and we do not link it to any vaccine record. Please don’t include medical details in the form or in emails to us.
5. How we use your information
- To mail the stickers you asked for, and to contact you about your request if needed.
- If you checked the box, to send occasional ivaccinated.org updates. Every email includes a way to unsubscribe.
- To report results in aggregate (for example, how many stickers went to each state). These reports do not identify individuals.
- To operate, protect and improve the Site, prevent abuse, and meet legal obligations.
6. What we don’t do
- We don’t sell or rent your personal information, or share it for advertising.
- We don’t use advertising or analytics cookies, tracking pixels or retargeting.
- We don’t show ads.
- Fonts and images are served from this Site, not from third-party services.
If this changes, for example if we add privacy-friendly analytics, we will update this notice first.
7. Who handles your information
AHIS handles sticker requests itself. Requests are processed and mailed by AHIS staff and AHIS contractors. They may use your information only to fulfill requests for AHIS, and they are bound by confidentiality.
We also use a small number of service providers. They process information only on our behalf and according to our instructions:
- Netlify, Inc. (United States): website hosting, server logs, form processing and form notification emails.
- Microsoft Corporation (Microsoft 365): email and secure storage used to receive and process requests.
- Postal and shipping carriers, such as the U.S. Postal Service, which receive your name and address to deliver your stickers.
We may also disclose information if required by law, or to protect the rights, safety or security of AHIS, the Site or others.
8. How long we keep it
- Mailing addresses: deleted within 90 days after your stickers ship.
- Names and emails: deleted with your mailing address, unless you opted in to updates. In that case we keep your name and email until you unsubscribe.
- Messages: as long as needed to respond and keep reasonable records.
- Server logs: according to our hosting provider’s standard retention.
9. Your choices and rights
You can unsubscribe at any time using the link in any email or by writing to info@ahis.org. You can also ask us to access, correct or delete the personal information we hold about you. We will respond within a reasonable time and in line with applicable law.
10. Do Not Track and California privacy
Do Not Track. We do not track visitors across websites or over time, and we do not allow third parties to do so on this Site. The Site works the same way whether or not your browser sends a “Do Not Track” or Global Privacy Control signal.
California residents. This notice is intended to meet the California Online Privacy Protection Act. We do not sell or share personal information, and we do not disclose it to third parties for their own marketing purposes.
11. Children
The Site is intended for adults. It is not directed to children under 13, and we do not knowingly collect personal information from them. If you believe a child has submitted information to us, contact us and we will delete it.
12. Security
We take reasonable measures to protect personal information, including encrypted (HTTPS) connections and limited access to requests. No method of transmission or storage is completely secure, so we cannot guarantee absolute security.
13. Links to other websites
The Site links to other websites, including state and territorial immunization registries, the CDC, medical societies and partners. When you look up your vaccine record, you do so directly with your state or territory. Their privacy practices are their own, and we encourage you to read their policies.
14. Changes to this notice
We may update this notice from time to time. The date at the top shows when it last changed. Material changes will be highlighted on the Site.
