1. About these terms

These Terms of Use (“Terms”) apply to ivaccinated.org (the “Site”), operated by Alliance for Health Implementation Science, Inc. (“AHIS,” “we,” “us”). By using the Site, its graphics or its stickers, you agree to these Terms. If you don’t agree, please don’t use the Site.

2. Not medical advice

The Site helps you find your own vaccine record and points to vaccine schedules published by medical societies and the CDC. It is general information, not medical advice, and not a substitute for advice from a qualified healthcare provider. Talk with your provider about which vaccines are right for you or your family. If you think you or someone else is having a medical emergency, call 911.

3. Vaccine records and registry links

Vaccine records are kept by state, territorial and local immunization registries, not by AHIS. The Site links to those registries’ public tools so you can request or view your own record. We don’t control those websites, their availability, their requirements or the accuracy of the records they hold. If a record is missing or wrong, contact the registry or the provider who gave the vaccine.

We check every registry link at least once a year, but registries change their websites without notice. If a link is broken or out of date, please let us know.

4. The “How easy does your state make it?” map

The map and the “No online lookup yet” list summarize the options each registry publicly offers adults to get their own record, as we found them on the date shown on the Site. They describe access, not the quality of any registry or public health program, and they are not legal or official findings. Registries and agencies that believe a listing is out of date can write to info@ahis.org. We review corrections promptly and update the Site when a listing changes.

5. Guidelines from medical societies and CDC

Links to vaccine schedules are provided for convenience. Each schedule belongs to the organization that publishes it, and those organizations do not review or endorse the Site. Listing an organization does not mean it endorses AHIS, and AHIS does not endorse any specific vaccine product, brand or manufacturer.

6. The sticker, mark and social toolkit

The “I Vaccinated” sticker, the ivaccinated.org name and the social media graphics are made available by AHIS. You may download, post, share and print them, and wear the stickers, for personal and non-commercial awareness use, provided that you:

  • do not alter the sticker or mark, except to resize it;
  • do not use them to advertise or sell any product or service, or in a way that suggests AHIS endorses you, your organization or any product;
  • do not sell the stickers or graphics, or put them on merchandise for sale; and
  • do not use them in any way that is misleading, unlawful or harmful.

Clinics, pharmacies, health systems and community organizations may hand out stickers we send them to the people they serve. All other rights are reserved. For any other use, contact info@ahis.org.

7. Free sticker requests

  • Stickers are free while supplies last, and only to U.S. mailing addresses.
  • Individual requests are limited to one per household. Provider and pharmacy requests receive one roll unless we agree otherwise.
  • We may decline, limit or delay any request, including duplicate, bulk, automated or suspicious requests, at our discretion.
  • Delivery times are estimates. We are not responsible for mail that is lost or delayed by a carrier.
  • When you request stickers for a provider or pharmacy, you confirm the address is correct and that you’re comfortable with us mailing to it.

8. Acceptable use

Please don’t misuse the Site, for example by attempting to disrupt it, gain unauthorized access, submit spam, false or automated requests, or impersonate another person or organization.

9. Links to other websites

The Site links to websites operated by others, including government agencies, medical societies and partners. We are not responsible for their content, accuracy, availability or practices.

10. No warranty

The Site, its information and its materials are provided “as is” and “as available,” without warranties of any kind, express or implied, to the fullest extent permitted by law. We do not guarantee that the Site will be uninterrupted, error-free or free of harmful components, or that any registry link or listing is current at every moment.

11. Limitation of liability

To the fullest extent permitted by law, AHIS and its directors, staff, volunteers and partners will not be liable for any indirect, incidental, special or consequential damages, or for any loss arising from your use of, or inability to use, the Site, its links, its materials or its stickers.

12. Changes

We may change the Site, its materials or these Terms at any time. The date at the top shows when these Terms last changed. Continuing to use the Site after a change means you accept the updated Terms.

13. Governing law

These Terms are governed by the laws of the State of Delaware, without regard to its conflict-of-laws rules. Any dispute relating to the Site or these Terms will be brought in the state or federal courts located in Ventura County, California, and you consent to their jurisdiction.
